Medical Referral

Prior to Referral

In most cases a referral for medical tuition is a result of a young person having struggled to attend school for a number of weeks and months due to a mental health or physical illness. When a referral is being considered by the school it is best practice to arrange a family and multi-agency meeting inviting a representative from Tor School. This allows everyone involved to discuss and plan the best way to reengage a young person with their education and whether a referral to Tor School is appropriate and has the support of the medical professionals involved. If the school then go ahead with a referral to Tor School we already have a good grasp of the young person’s issues and so can plan an effective package for them. It can be more difficult to do this from just the paper referral with no other contact from the school.

If a family and multi-agency meeting has not been arranged prior to the referral being made we will ask you to do this after the case has been discussed at Allocations Panel if the referral has been accepted. This will enable us to plan appropriate provision for the child or young person.

Once a multi professional meeting has been held to agree that a medical referral is appropriate please read these supporting guidelines for details on what makes a complete medical referral. It’s the referring school’s responsibility to complete the relevant forms, and gather the medical evidence together before submitting the referral.

All medical referrals will be discussed at the monthly Allocations and Partnership meetings and you will be informed of the outcome.

Outcome and actions following the Allocations and Partnership meeting will be sent to the referring school

Referrals without medical evidence will be returned. It is the referring schools responsibility to ensure that we receive the ‘Medical Information’ form.

 

It is important that the supporting medical practitioner form gives clear guidance on the number of hours of education the student can access each day and the environment in which lessons should take place.

We work only with those students who are not currently attending school provision, and who will not be able to attend school even with adaptations to timetables and support being given by the school.

 

Outcomes

Once a referral has been discussed by the allocations panel the outcome will be sent to us and the referring school, and the necessary arrangements for provision will be made.
